If the birth or death occurred in Grant County you can come in to our office and obtain the certificate immediately or you can send a request through the mail.
To send a request for birth certificate you can click on the "forms" tab on the previous page to print an application or you can write a letter in which you will need to provide the following information; full birth name, date of birth, full name of father, and full maiden name of mother, your legal address and daytime telephone number. WHETHER YOU USE THE APPLICATION FORM OR WRITE A LETTER you will also need to send a photocopy of your drivers license or state issued identification card, a stamped, self-addressed return envelope, and a money order. The fee for a certified birth certificate is $10.00 per copy. Please note that we do not accept personal checks.
To send a request for a death certificate you will need to provide the name of the decedent, your relationship to the decedent, the approximate date of death, a photocopy of your drivers license or state issued identification card, a stamped, self-addressed return envelope and a money order.
